Key Responsibilities
  • Assist clients with daily tasks and provide support with self-care skills, social skills, and independent living skills.
  • Establish rapport with clients and help them identify their needs to develop appropriate rehabilitation goals.
  • Provide internal and/or community referrals as necessary to support clients' needs.
  • Work collaboratively with a multidisciplinary team to develop and implement individualized rehabilitation plans.
  • Maintain accurate and timely documentation of client interactions and progress.
Requirements
  • High school diploma or equivalent required; BA/BS in Human Services or related field preferred.
  • Ability to obtain additional certifications/trainings as required for certain programs.
  • Valid state driver's license with 3 or fewer points at time of hire.
  • Ability to physically assist clients with daily tasks.
  • Excellent organizational, communication, and time management skills.
Working Conditions

This is a full-time position that requires the ability to work a variety of shifts, including days, evenings, and weekends, as well as holidays.
