Your Role in Making a Difference

The Social Worker at the Bachelor's level is essential in evaluating patients from a psychosocial standpoint and offering ongoing support with community and facility resources, family engagement, and education. This role also addresses matters such as legal guardianship and forensic issues. Responsibilities include conducting educational sessions, performing assessments of strengths and needs, and maintaining communication between the treatment team and families, guardians, and community service providers. Furthermore, this position coordinates discharge processes to ensure patients receive the appropriate care and support.

Key Responsibilities

  • Delivers comprehensive therapeutic services and interventions to address discharge barriers.
  • Oversees all aspects of discharge planning and documentation, advocating for patients within the framework of clinical training and professional standards.
  • Enhances patients' abilities to meet their own needs, utilizing patient interactions as opportunities for therapeutic engagement.
  • Collaborates with colleagues and seeks guidance when it benefits patient care.
  • Participates in weekly departmental meetings, contributing to problem-solving and sharing ideas.

Qualifications & Requirements

Education

  • Bachelor's degree from an accredited institution is required.

Experience

  • At least two (2) years of practical experience in social services or a related field, particularly with the chronically mentally ill population.

Additional Information

This position is a full-time role with daytime hours, Monday through Friday, and does not require weekend work.
