About the Role

We are seeking a skilled Case Manager to join our team at Atlantic Rehabilitation Institute. As a Case Manager, you will play a vital role in the care and recovery of our patients.

Key Responsibilities
  • Develop and implement individualized care plans to achieve patient goals and outcomes.
  • Collaborate with interdisciplinary teams to identify barriers to care and develop solutions.
  • Document patient progress and outcomes in accordance with CMS guidelines.
  • Communicate with patients and caregivers to ensure informed decision-making and timely discharge planning.
  • Coordinate weekly patient care team conferences to refine treatment plans and achieve patient goals.
  • Review patient assignments and identify potential missed comorbid conditions, communicating findings to the HIM team.
Requirements
  • Current licensure as a Registered Nurse, Social Worker, or Healthcare professional (Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist).
  • Certification in Case Management or Rehabilitation Nursing preferred.
  • Minimum of 2 years experience in an inpatient setting, with acute/rehabilitation hospital experience preferred.
  • Effective oral and written communication skills in English, with additional languages a plus.
  • Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.
