Job Summary

Porchlight, Inc. is seeking a skilled Case Manager to join our team in providing supportive services to veterans in need. As a Case Manager, you will work closely with participants to create individualized service plans, promote stable income, and facilitate a successful transition from our program into permanent housing.

Key Responsibilities

Case Management:

  • Collaborate with the VTHP Peer Specialist and VA Liaison to screen potential program participants.
  • Develop comprehensive, client-driven case plans that identify short-term and long-term goals.
  • Monitor progress towards goals in weekly or biweekly meetings with participants.
  • Empower participants to become involved in their own planning and goal setting.
  • Refer participants to appropriate resources to meet their goals.

Recordkeeping & Reporting:

  • Enter and exit all participants into the Homeless Management Information System (HMIS).
  • Document and report all incidents involving emergency services, law enforcement interactions, and participant issues.
  • Notify proper agencies/providers of significant changes in behavior indicating instability.

Property Management:

  • Perform quarterly housekeeping inspections with the VTHP Peer Specialist.
  • Conduct quarterly fire drills and maintain up-to-date documentation on evacuation plans.

Personal Development:

  • Maintain professional behavior and boundaries at all times in relationships with participants, staff, and community agencies.
  • Meet biweekly for individual supervision with the Director of Supportive Services.

Qualifications:

  • Bachelor's Degree in Social Work or related field desired, or equivalent knowledge and experience.
  • Military background/experience is preferred, but not required.
  • Experience working with individuals with mental health and substance use-related concerns preferred.

Requirements:

  • Full Time, 40 hours per week
  • Monday - Friday, 8:30-5:00
