PRIMARY RESPONSIBILITIES:

Deliver guidance and support in activities that foster autonomy for individuals with intellectual and developmental disabilities who opt for self-directed services. The Self-Directed Program is designed to provide tailored care to individuals, ensuring their daily needs are met and empowering them to achieve their highest potential.

KEY DUTIES:

1. Assist individuals in achieving specific outcomes as outlined in their personalized Self-Directed Plans. Responsibilities may encompass personal care, community engagement, financial management, advocacy, employment support, and respite services.

2. Actively seek opportunities to address individuals' psychosocial needs through both planned care and spontaneous interactions.

3. Deliver reliable, safe, and high-quality services with thoroughness and compassion.

4. Employ a person-centered approach in all service delivery.

5. Adhere to universal precautions, infection control measures, and safety standards in all tasks and activities.

6. Complete paper time sheets/invoices and utilize web-based timekeeping promptly in accordance with payroll protocols.

7. Maintain communication with supervisors and the Circle of Support regarding any necessary updates or revisions to individuals' habilitation plans.

8. Effectively communicate with individuals with intellectual and developmental disabilities, their support circles, brokers, fiscal intermediaries, service coordinators, supervisors, interagency professionals, community service providers, and families.

9. Collaborate consistently with all team members, prioritizing the preferences of individuals and their families to fulfill their needs and aspirations.

10. Uphold the dignity, privacy, property, beliefs, and culture of the individuals served.

11. Accurately document and maintain records of interactions with individuals.

12. Provide appropriate interventions during crisis situations and promptly notify the relevant authorities of any allegations of abuse or neglect.

13. Attend mandatory training sessions.

14. Offer transportation to individuals as required, adhering to safety protocols during transit.

15. Exhibit flexibility in scheduling based on the needs of assigned individuals.

16. Demonstrate reliability by arriving on time and being prepared for work.

17. Perform additional duties as assigned by the individual receiving services or the supervising staff.

QUALIFICATIONS:

EDUCATION:

High School Diploma or equivalent.

EXPERIENCE:

A minimum of one year of practical experience working with individuals with intellectual and developmental disabilities is preferred. A combination of relevant experience and education may be considered at the discretion of the Executive Director.
