LPN - Academic Accountable Care Unit Nurse

The Licensed Practical Nurse (LPN) plays a vital role in providing patient care and administrative support in an environment that is sensitive to each person's physical, emotional, social, and spiritual needs. Under the direction of a Provider or RN, the LPN will work to ensure that patients receive the highest quality care.
Responsibilities:
• Directs unlicensed staff in work-related activities under the supervision of a registered nurse.
• Demonstrates the ability to manage the patient assignment effectively.
• Uses assessment skills to collect data and collaborates with RN to discuss next steps in patient care.
• Communicates pertinent clinical information to appropriate members of the patient care team.
• Keeps patient/family apprised and updated.
• Provides patient/family education under the direction of the registered nurse.
• In conjunction with the physician and RN, identifies discharge needs, participates in planning, and completes continuity of care forms as appropriate.
• Collaborates with attending physicians, house staff, registered nurse, and other healthcare disciplines regarding patient care.
• Performs treatments and procedures according to approved standards, policies, procedures, and guidelines.
• Verifies and implements physician/other licensed independent practitioner's orders.
• Administers medications according to the organization's standards, policies, procedures, and/or guidelines.
• Initiates appropriate emergency procedures within the scope of practice under the direction of the RN/physician.
• Practices proper patient identification procedures.
• Maintains equipment appropriate to the care environment.
• Establishes priorities for the delivery of nursing care and meets patient needs.
• Documents accurately and completely pertinent information/data in accordance with documentation standards, policies, procedures, and/or guidelines.
• Utilizes computer systems effectively, including ATK Systems, appropriate to the area.
Requirements:
• Successful completion of an approved Pharmacology Course with NAPNES Certification, OOPNES Certification, or Ohio Board of Nursing Card.
• Ability to operate a motor vehicle required; possesses a valid driver's license from the State of Ohio and has access to a functional automobile during work hours.
• Requires use of a personal vehicle with valid auto insurance.
• Current clinical experience, preferably in community home health or hospice setting.
• Criminal background check and fingerprinting completed as required by Ohio law.
• Graduate of an accredited School of Practical Nursing.
• Current license in good standing to practice as a Licensed Practical Nurse (LPN) in the State of Ohio.
• Current BLS/CPR and maintain current certification; IV certification preferred.
