LPN - Academic Accountable Care Unit NEW

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Mount Carmel St. Ann's. As an LPN, you will provide administrative support and patient care in an atmosphere sensitive to each person's physical, emotional, social, and spiritual needs.
Responsibilities- Directs unlicensed staff in work-related activities under the supervision of a registered nurse.
- Uses assessment skills to collect data and collaborate with RN to discuss next steps in patient care.
- Provides patient/family education under the direction of the registered nurse.
- Identifies discharge needs, participates in planning, and completes continuity of care forms as needed.
- Collaborates with attending physicians, house staff, registered nurse, and other healthcare disciplines regarding patient care.
- Successful completion of an approved Pharmacology Course with NAPNES Certification, OOPNES Certification, or Ohio Board of Nursing Card.
- Ability to operate a motor vehicle required; possesses a valid driver's license from the State of Ohio and has access to a functional automobile during work hours.
- Current clinical experience, preferably in community home health or hospice setting.
- Criminal background check and fingerprinting completed as required by Ohio law.
