About the Job

We are looking for a talented Professional Care Coordinator to join our team at UPMC. In this role, you will be responsible for providing high-quality patient care and coordinating with multidisciplinary teams to achieve patient-centered care.

As a key member of our care delivery team, you will set the standards for the level and quality of patient care and work closely with physicians, other healthcare providers, patients, and their families to achieve desired patient outcomes.

Key Responsibilities:

  • Provide exceptional patient care and education
  • Collaborate with multidisciplinary teams to achieve patient-centered care
  • Participate in quality improvement initiatives to enhance patient outcomes

Requirements:

  • Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement
  • Bachelor's degree in Nursing (BSN) preferred
  • Minimum six months experience as a registered nurse

Estimated Salary: $90,000 - $125,000 per year
