  • A Registered Nurse Care Coordinator plays a vital role at AHF by applying strong clinical nursing skills and a comprehensive understanding of the Ryan White Program within their designated area. With a compassionate and knowledgeable approach, you will collaborate with our physicians, other healthcare providers, the Care Management team, and the Healthcare Center team to ensure that every Ryan White patient receives the necessary support and access to available resources. Working directly within the Healthcare Center, you will engage with patients to conduct Needs Assessments, develop Care Plans, and provide ongoing education to clients, enabling them to understand their diagnoses and medications. Your teamwork will facilitate a holistic approach to meet all aspects of your patient's health and psychosocial needs. In this manner, you will empower patients to better comprehend their health status and work towards achieving their healthcare objectives.

A Registered Nurse Care Coordinator enhances the reputation of AHF by employing core nursing principles. You will meet patients where they are, identify their strengths and opportunities for growth, and assist them in setting realistic goals.

With your collaboration and engagement skills, you will embody the essence of the Ryan White Care Act - serving as a safety net for uninsured HIV/AIDS patients - to our most vulnerable population.

By establishing Needs Assessments, Care Plans, referrals, and support, your contributions will directly impact AHF's success in fulfilling the requirements of our Ryan White contract.

Beyond the contract's scope, your efforts will contribute to measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

The Registered Nurse Care Coordinator will work as part of a close-knit team to provide coordinated healthcare interventions that assist patients in self-managing their HIV.

This role will also facilitate self-management, including making and keeping healthcare appointments, adhering to prescribed ARTs and other medications, and communicating with healthcare providers to coordinate care for comorbid chronic or disease conditions.


Licenses and Certifications:

Bachelor's degree in Nursing (BSN) or Licensed Practical Nurse (LPN) with a minimum of 1 year of Case Management experience.
