YOUR ROLE IN OUR MISSION


As a Registered Nurse Care Coordinator, you will play a vital role at AHF by applying your robust nursing expertise and knowledge of the Ryan White Program in your designated area.

With a compassionate and informed approach, you will collaborate with physicians, care management teams, and healthcare center staff to ensure that every Ryan White patient receives the necessary support and access to resources.

This will involve providing education and follow-up care for clients.

Working directly within the healthcare center, you will engage with patients to conduct Needs Assessments, create Care Plans, and offer ongoing education to help them understand their health conditions and medications.

Collaborating with your team will allow you to address all aspects of your patients' health and psychosocial needs.

In doing so, you will empower patients to gain a better understanding of their health status and work towards achieving their healthcare objectives.


As a Registered Nurse Care Coordinator, you will enhance AHF's reputation by employing fundamental nursing principles, meeting patients where they are, identifying strengths and opportunities for improvement, and assisting them in setting realistic goals.

Your collaboration and engagement skills will be essential in bringing the essence of the Ryan White Care Act - a safety net for uninsured HIV/AIDS patients - to our most vulnerable communities.

By establishing Needs Assessments, Care Plans, referrals, and support systems, your contributions will directly impact AHF's success in fulfilling the requirements of our Ryan White contract.

Beyond contractual obligations, your efforts will also support measures set forth by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.

You will serve as a passionate advocate for our key initiatives.

The Registered Nurse Care Coordinator will be part of a cohesive team dedicated to providing coordinated healthcare interventions that empower patients to manage their HIV effectively.

This role will also involve promoting self-management, including scheduling and attending healthcare appointments, adhering to prescribed ARTs and medications, and communicating with healthcare providers to coordinate care for any comorbid conditions.


Qualifications:

A Bachelor's degree in Nursing (BSN) or a Licensed Practical Nurse (LPN) with at least one year of Case Management experience is required.

Familiarity with Ryan White Services is preferred.
