Job DescriptionWe are seeking a dedicated and compassionate Care Coordinator to join our team. The ideal candidate will have experience providing direct care services to individuals with disabilities or those aged 65 and older, possess strong communication and organizational skills, and be committed to delivering high-quality care.
Responsibilities- Provide direct care services to clients in accordance with their individual needs and care plans.
- Collaborate with families, healthcare professionals, and other stakeholders to ensure comprehensive client care.
- Maintain accurate records of client care, progress, and any concerns.
- Adhere to all applicable licensing requirements and regulations.
- Minimum of four years of experience providing direct care services to individuals with disabilities or those aged 65 and older, or equivalent education and experience in a related field.
- Ability to learn, understand, and implement Chapter 245 D licensing requirements.
- Excellent written and verbal communication skills.
- Strong organizational skills and the ability to manage multiple priorities effectively.
- A baccalaureate or associate degree in human services, education, or health is preferred.
$24.00 per hour
