Private Duty Nurse LVN

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide high-quality, patient-centered care to medically fragile children and adults in the comfort of their own homes.
Key Responsibilities- Deliver Quality Patient Care: Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders.
- Assess and Coordinate Care: Continuously observe and assess patient condition and care needs, reporting changes in condition to the supervisor and/or physician as appropriate.
- Document Care Provided: Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered.
- Participate in Care Planning: Participate in the development and implementation of the nursing care plan, taking into account the patient's medical history, current condition, and treatment goals.
- Provide Education and Support: Educate patients and their families on disease management, self-care techniques, and prevention strategies, promoting independence and empowerment.
- Education and Licensure: Graduate of an accredited school of nursing and current, unrestricted state license as a Licensed Nurse in the state of practice.
- Certifications and Experience: Current CPR certification and at least six months of recent experience as a Licensed Nurse in a clinical care setting, preferably in home health or pediatrics.
- Skills and Abilities: Strong communication and interpersonal skills, with the ability to work independently and as part of a team. Must be able to lift 50 pounds and have visual and hearing acuity.
