We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Private Duty Nurse. As a key member of our home health care team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.
Key Responsibilities- Implement and manage individualized care plans in accordance with physician orders and patient needs.
- Conduct thorough assessments and evaluations to identify patient care requirements and develop effective care strategies.
- Administer medications, treatments, and therapies as prescribed by physicians and document all care activities in a timely and accurate manner.
- Provide education and training to patients and their families on disease management, self-care techniques, and prevention strategies.
- Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive patient care.
- Participate in ongoing education and training to maintain current knowledge and skills in areas such as infection control, medication management, and patient safety.
- Graduate of an accredited school of nursing with a current, unrestricted license as a Licensed Nurse in the state of practice.
- Current CPR certification and demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- One year of experience as a Licensed Nurse in a clinical care setting, with a preference for home health experience.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to travel to patient homes and work in a variety of environments, with possible exposure to allergens and other conditions.
