Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our interdisciplinary team to develop and implement personalized care plans that meet their unique needs.

Responsibilities
  • Implement and coordinate care plans in accordance with physician orders and patient needs.
  • Conduct thorough patient assessments and document care provided in a timely and accurate manner.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Provide education and training to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Participate in ongoing education and training to maintain knowledge of best practices and industry developments.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
