Housing Support SpecialistPosition Summary:
The Housing Support Specialist plays a vital role in guiding participants through various emergency housing, safe haven, and supportive housing programs. This position focuses on providing trauma-informed care and connecting individuals with essential resources to secure permanent housing and prevent future homelessness.
Key Responsibilities:- Provide direct support and guidance to participants, adhering to a harm reduction model and client-centered approach.
- Conduct comprehensive assessments of each participant's needs within 72 business hours of enrollment.
- Develop individualized service plans with clear goals and timelines for transitioning participants from temporary housing to permanent supportive environments.
- Connect participants with appropriate medical, mental health, and legal services through referrals and scheduling appointments.
- Maintain up-to-date knowledge of available resources, benefits, and services for homeless individuals and provide accurate information to participants.
- Collaborate with community partners and external agencies to ensure seamless support for program participants.
- Document all interactions, progress, and case management activities meticulously.
- Participate in meetings and training sessions to enhance skills and stay informed about best practices.
- High School Diploma or GED required; Bachelor's degree in human services preferred.
- Minimum of three years of experience working with homeless individuals, mental health, and/or addiction treatment services.
- Strong verbal and written communication skills.
- Proficiency with Microsoft Office Suite and internet use.
- Demonstrated ability to work effectively with a diverse population.
- Excellent organizational and time management skills.
