RN Care Manager

We are seeking an experienced RN Care Manager to join our team at York Hospital. As a key member of our care management department, you will play a vital role in coordinating concurrent utilization management activities hospital-wide.
Key Responsibilities- Conduct thorough assessments of patient needs and identify required resources for successful transitions to the next care setting.
- Collaborate with interdisciplinary teams to develop and implement effective care plans.
- Utilize excellent communication skills to facilitate seamless communication with patients, families, healthcare teams, and payors.
- Secure necessary resources, including durable medical equipment and health insurance authorizations, to support patient care.
- Participate in length of stay management for inpatient hospital populations.
- Associate or Bachelor degree-prepared Registered Nurse with 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, or utilization review.
- Certification by a nationally recognized case management or utilization review organization.
