Job Summary: As a Community Advocate Specialist, you will provide essential support to individuals with developmental disabilities, ensuring they receive the necessary resources and services to lead fulfilling lives. Your primary objectives include establishing strong relationships, identifying individual needs, and securing ongoing support services.

Key Responsibilities:
  • Advocate on behalf of individuals, providing education and resource information to empower self-determination
  • Collaborate with providers to identify and secure ongoing support services
  • Establish and monitor Person-Centered Plans, outlining outcomes and support needs
  • Meet state-mandated requirements for all duties of the Service Coordinator role

Requirements:
  • Bachelor's degree in human services or a non-human services field with relevant experience
  • Experience working with people with developmental disabilities (preferred)
  • Excellent communication and interpersonal skills
