Job Overview

Social Work p.r.n. is seeking a skilled Medical Social Worker for a temporary, full-time position in Columbia, Maryland. About the Role

The successful candidate will be responsible for conducting thorough psychosocial assessments to identify patients' emotional, social, and environmental needs. They will collaborate with healthcare teams to develop patient-centered care plans, provide short-term counseling and crisis intervention, and assist with discharge planning to ensure a smooth transition. Additionally, they will connect patients and families with community resources and educate them on topics such as advance directives and coping strategies.

Key Responsibilities

  • Conduct psychosocial assessments for new patients to identify their needs
  • Collaborate with healthcare teams to develop patient-centered care plans
  • Provide short-term counseling and crisis intervention to patients and families
  • Assist in discharge planning to coordinate services for a smooth transition
  • Connect patients and families with community resources
  • Educate patients and families on various topics

Requirements

  • Master's degree in Social Work (MSW) from an accredited program; clinical licensure (LMSW, LICSW)
  • Understanding of psychosocial aspects of aging, illness, and care transitions

What We Offer

We offer a competitive salary range of $65,000-85,000 per annum, depending on experience, plus excellent benefits and opportunities for growth and development.
