Community Choice Behavioral Health is seeking a skilled Care Coordinator - Behavioral Health to join our team in Saco, Maine.

Job Description

We provide Community Integration Case Management services and Outpatient Therapy with an emphasis on Trauma-Informed care practices. As a Care Coordinator, you will work with diverse populations, providing individualized, creative, and flexible services infused with evidence-based and evidence-informed strategies.

This is a fee-for-service position with flexible hours, offering opportunities for career advancement and professional development. Experience working with populations with co-occurring disorders/serious mental illness/behavioral challenges is required.

Key Responsibilities:

  • Provide exceptional Community Integrated Case Management services
  • Develop and implement treatment plans tailored to each client's needs
  • Collaborate with healthcare professionals to ensure comprehensive care

Requirements/Duties:

  • Must possess current valid MHRT/C certification
  • Experience with a trauma-informed recovery-focused philosophy/ability to respond appropriately to crisis situations
  • Good organizational and computer skills; previous experience with electronic medical records preferred
  • Motivated self-starter with the ability to independently manage caseloads
  • Current Maine Driver's License/vehicle insurance as required by state law
