As a Care Coordinator at Next Move Healthcare, you will play a vital role in ensuring the well-being of our patients undergoing dialysis treatment. Your primary responsibility will be to provide comprehensive psychosocial support, ensuring that their rights, dignity, and overall quality of care are maintained in a safe environment.

The ideal candidate for this position should possess a Master's degree from an accredited school of social work and state licensure (or eligibility). Additionally, they should have a minimum of one year of clinical social work experience, preferably in an ESRD setting. CPR certification is preferred but not mandatory.

You will be working closely with the Interdisciplinary Team (IDT) to ensure seamless coordination with service providers and participate in patient care planning. As part of your duties, you will also assist patients with financial aid, insurance, transportation, and home health care services, while helping improve community resources.

We offer a competitive salary range of $60,000 - $80,000 per annum, depending on experience, along with a comprehensive benefits package including medical/Rx insurance allowance, dental and vision coverage, paid vacation and sick time, 8 paid holidays, 401(k) plan, company-paid life insurance, flexible spending accounts, short-term/long-term disability, and employee assistance program (EAP).
