Director of Resident Services

Avante at Melbourne, Inc. is seeking a highly skilled and compassionate Director of Resident Services to join our team. As a key member of our Social Services Department, you will be responsible for providing social services to residents in our long-term care facility, ensuring their medically related psycho-social needs are met and maintained.
Job Summary:The Director of Resident Services will act as a liaison and representative of the residents' social interests, planning, organizing, developing, and directing the overall operation of the Social Services Department. This role requires a strong understanding of federal and state standards governing long-term care facilities and the ability to ensure that the medically related psycho-social needs of residents are met and maintained on an individual basis.
Key Responsibilities:- Maintain contact with facility caregivers to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning
- Timely and accurate assessment and completion of all social services documentation in the medical record
- Assist staff in informing residents and their designated representatives about their health status and healthcare choices
- Make referrals and obtain services from outside entities as necessary
- Assist residents with necessary referrals for financial and legal matters
- Provide discharge planning services
- Arrange provision of needed counseling or psychiatric services
- Identify and seek ways to support residents' individual needs
- Promote actions by staff that maintain or enhance each resident's dignity
- Assist residents and their families with Advance Directives
- Provide alternatives to drug therapy by understanding and communicating with staff
- Identify and address residents who are grieving and arrange for necessary services
- Assist with Resident abuse and neglect investigations and grievances
- Participate in all required staff meetings
- Assist the administrator with interviewing, providing oversight, and ensuring compliance with state and federal regulations
- Bachelor's Degree in Social Work or Human Services preferred
- Minimum of three years' experience in Long Term Care
- At least one year of supervised Social Work experience in a Health Care setting working directly with individuals
- Ability to stand, walk, sit, use hands, reach, climb stairs, balance, lift, and move equipment or residents
- Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus
- Ability to work in a constant state of alertness and in a safe manner
- Works beyond normal working hours as necessary
