Purpose and Qualifications

To conduct and coordinate the development and completion of the resident assessment in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility, and as may be required by the Executive Director, to ensure that the highest degree of quality care is maintained at all times.

Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state. Must be able to read, write, speak, and understand the English language. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

Responsibilities
  1. Conduct and coordinate the development and completion of the resident assessment, minimum Data Set (MDS) and Care Planning in accordance with current rules, regulations, and guidelines that govern the resident assessment in Avante facilities.
  2. Ensure that all MDS Assessments are completed and transmitted to the State repository according to the regulations in the RAI Manual.
  3. Ensure that all residents meet skilled care coverage criteria per governmental or other regulatory requirements.
  4. Coordinate the review and revision of the resident's care plan by the interdisciplinary team after each quarterly review or other assessment, assuring that the care plan is evaluated and revised each time an assessment is done or when there is a change in the resident's status.
  5. Monitor Quality Measures Report on a monthly basis with particular emphasis on those measures impacting the Federal Five Star System to ensure that residents in the numerator of each measure are coded correctly according to the RAI guidelines.
  6. Assist the nursing staff in encouraging the resident and his/her family to participate in the development and review of the resident's plan of care.
  7. Ensure that all personnel involved in providing care to the resident are aware of the resident's care plan and that nursing personnel refer to the resident's care plan prior to administering daily care to the resident.
  8. Participate in facility surveys (inspections) made by authorized government agencies.
  9. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
Working Conditions

Works in office area(s) as well as throughout the nursing service area (i.e., nurses' stations, resident rooms, etc.). Moves intermittently during working hours and is subject to frequent interruptions. Is involved with residents, family members, personnel, visitors, government agencies and personnel, etc., under all conditions and circumstances.

Physical Job Requirements

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell.

The employee must occasionally lift and/or move up to 20-50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Education and Experience

Must possess, as a minimum, a Nursing Degree/Diploma from an accredited nursing school, college or university. Preferred, as a minimum, two (2) years of experience in a hospital, nursing care facility, or other related health care facility.
