Job Summary

Elevance Health is seeking a highly skilled LTSS Service Coordinator to join our team. As a key member of our care coordination team, you will be responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs.

Key Responsibilities
  • Conduct face-to-face program assessments to identify individual needs and develop comprehensive Person-Centered Support Plans (PCSPs)
  • Collaborate with individuals, families, and care teams to ensure seamless care coordination and support
  • Identify and address gaps in care, including potential clinical health care needs, and coordinate with clinical healthcare management and interdisciplinary teams
  • Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of long-term services and supports
  • Document short- and long-term service and support goals in collaboration with individuals and their chosen care teams
  • Identify individuals who may benefit from alternative levels of service or other waiver programs
  • Participate in process improvement initiatives and serve as a mentor, subject matter expert, or preceptor for new staff
Requirements
  • Bachelor's degree in a healthcare-related field or equivalent experience
  • Minimum of 2 years of experience working with a social work agency or equivalent experience
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong analytical and problem-solving skills
