Job Title: LTSS Service Coordinator-Dallas County, IA

We are seeking a highly skilled and compassionate LTSS Service Coordinator to join our team in Dallas County, IA. As a key member of our care coordination team, you will play a vital role in ensuring that individuals with long-term services and supports needs receive the highest quality care and support.

Job Summary:

The LTSS Service Coordinator will be responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs. This includes conducting assessments, developing comprehensive Person-Centered Support Plans (PCSPs), and interfacing with medical directors and interdisciplinary care rounds to support the development of fully integrated care plans.

Key Responsibilities:
  • Conduct face-to-face program assessments using various tools with pre-defined questions to identify members with potential clinical healthcare needs.
  • Apply motivational interviewing techniques to evaluate, coordinate, and manage an individual's waiver needs, including LTSS/IDD and BH or PH needs.
  • Use tools and pre-defined identification processes to identify members with potential clinical healthcare needs and coordinate their cases with clinical healthcare management and interdisciplinary teams.
  • Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of long-term services and supports.
  • Document short- and long-term service and support goals in collaboration with the member's chosen care team, including caregivers, family, natural supports, service providers, and physicians.
  • Identify members who would benefit from alternative levels of service or other waiver programs.
  • May serve as a mentor, subject matter expert, or preceptor for new staff, assisting in the formal training of associates and participating in process improvement initiatives.
  • Submit utilization/authorization requests to utilization management with documentation supporting and aligning with the individual's care plan.
  • Report critical incidents to appropriate internal and external parties, including state and county agencies (Adult Protective Services, Law Enforcement).
  • Assist and participate in appeal or fair hearings, member grievances, appeals, and state audits.
  • Travel to worksites and other locations as necessary.
Requirements:
  • Requires a BA/BS degree and a minimum of 2 years of experience working with a social work agency; or any combination of education and experience that would provide an equivalent background.
  • Specific education, years, and type of experience may be required based on state law and contract requirements.
Preferred Skills, Capabilities, and Experiences:
  • BA/BS degree in a health care-related field is preferred.
