Job Summary:

  • Provide skilled nursing care and prescribed treatment to patients in their homes.
  • Develop and implement care plans that are goal-oriented and patient-centered.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.

Requirements:

  • BSN required; ADN candidates are required to obtain their BSN within three years of their start date.
  • Current DE RN license or compact state license.
  • 1 year of RN experience, with previous Home Healthcare and Med-Surgical experience preferred.
