About the Role

We are seeking an experienced Integrated Health Care Coordinator to join our team at SouthLight Healthcare. As an Integrated Health Care Coordinator, you will play a critical role in assisting participants in establishing a permanent primary care home, coordinating necessary referrals, and ensuring care coordination between SouthLight and primary care homes.

The ideal candidate will have QP status, experience working in a multi-disciplinary team in a medical setting, and strong organizational skills. You will also be comfortable working independently and in the community, with excellent interpersonal skills and ability to work collaboratively within a team.

In this role, you will establish relationships with primary care homes, schedule appointments, address social determinants of health barriers, and track CCBHC metrics. You will also work closely with the Medical Program Manager, Integrated Health Lead Physician, and other team members to ensure seamless care coordination.

Schedule:

Full-time days.

Pay:

Starting at $21.00+ per hour, based on experience.

Located in the Triangle area, we offer competitive pay and opportunities for professional growth and development.

The estimated salary range for this position is $45,000 - $60,000 per year.
