About the Organization:

Happiness House CP Rochester is a private nonprofit health, educational, residential, and human service organization serving communities in the Finger Lakes Region and the Greater Rochester Area.

Job Summary:

We are seeking a Community Habilitation Specialist to join our team. The successful candidate will provide training and support to individuals with disabilities to help them acquire daily living skills and achieve independence in their community living situations.

Key Responsibilities:
  • Impart agency philosophy and values to clients, families, and community resources.
  • Maintain confidentiality of information relating to individuals and their families.
  • Provide training and support to individuals with disabilities to help them acquire daily living skills.
  • Assist individuals with accessing community resources, including health and safety resources.
  • Act as a role model for individuals with disabilities by interacting with people in a manner that promotes successful community living.
  • Participate in case conferences and maintain necessary records and case notes.
Requirements:
  • High School Diploma or equivalent.
  • Knowledge of community resources and means of accessing these resources.
  • Valid NYS driver's license.
  • 6 months experience working with individuals with disabilities providing habilitation or rehabilitation and independent living skills training.
Work Environment:

This job operates in a home environment as well as in the community.
