Key Responsibilities:

  • Outreach and Engagement: Conduct community outreach to identify and engage potential clients, building trust and rapport with the target demographic.
  • Eligibility Assessment: Perform screening interviews to determine eligibility for POH services and present candidates to the POH team.
  • Case Management: Carry out comprehensive intake processes, including documentation and program explanation, and develop individualized case plans within one week of intake.
  • Support Services: Assist clients in accessing community resources, provide advocacy in various situations, and offer financial assistance as per program guidelines.
  • Monitoring and Evaluation: Maintain detailed case files, track client progress, and ensure confidentiality in line with established regulations.

Qualifications:

Skills: Candidates should possess strong communication abilities, a commitment to working with diverse populations, and a capacity for critical thinking and problem-solving.

Physical Requirements: The role involves various physical activities, including lifting up to 25 lbs, and may require local travel for client meetings.

Working Environment: The position is primarily office-based but includes community engagement, with some evening and weekend work as necessary.
