Key Responsibilities:
  • Provide administrative support to social workers and director, including patient admission and discharge processes.
  • Collaborate with patients, residents, and their families to provide support and resource information.
  • Respond to issues identified by patients, residents, and their families.
  • Facilitate patient/resident transfer within the nursing center to ensure a seamless transition.
  • Participate in the healthcare decision-making process under the direction of a qualified social worker and/or consultant.
