Social Worker

We are seeking a skilled Social Worker to join our team at MultiCare Health System. As a Social Worker, you will play a vital role in supporting patients and their families through complex issues, providing assessment, coordination, and advocacy services.
Key Responsibilities- Conduct thorough assessments of patients' social, emotional, and cognitive needs
- Develop and implement individualized treatment plans to address identified needs
- Collaborate with healthcare teams to ensure comprehensive care
- Provide education and support to patients and families on social work-related topics
- Master's degree in Social Work (MSW) from an accredited program
- Current licensure as a Licensed Social Worker (LSWAIC, LMHC, LMFT, LASW, LICSW) as required by department
- Experience working in a hospital or acute care setting, or equivalent social service agency
