Senior Care Coordinator

As a Nurse Care Manager at RiverSpring Living, you will play a vital role in care coordination, including in-home assessments, planning, facilitation, advocacy, and authorization of covered services to meet members' health needs while promoting quality, cost-effective outcomes.
Salary and Benefits
Annual salary: $105,000 - $115,000, plus eligible for a $20,000 sign-on bonus ($5,000 payable every six months, less taxes). Our comprehensive benefits package includes health insurance, retirement plans, paid time off, and professional development opportunities.
About the Role
In this critical position, you will collaborate with members and their families to maintain the most independent living situation possible. This includes assessing and closely monitoring members' needs and status, authorizing covered services, and coordinating care across different payers. Effective communication and collaboration with members, family members, caregivers, primary care practitioners, and the interdisciplinary team are essential to success in this role.
Qualifications and Requirements
To be successful in this role, you must have a current NYS RN License and registration, a Bachelor of Science in Nursing (BSN), and a minimum of 3 years of RN experience. Clinical expertise in geriatrics is preferred, and managed long-term care experience is also a plus.
