Job Summary

We are seeking a highly skilled and compassionate Social Services Specialist to join our team at Orono Commons in Orono, ME. As a key member of our interdisciplinary team, you will play a vital role in promoting the physical, mental, and psychosocial well-being of our patients/residents.

Key Responsibilities
  • Collaborate with the interdisciplinary team to identify and address the social service needs of patients/residents, ensuring their rights and well-being are protected and promoted.
  • Develop and implement individualized plans of care to address social service needs, working closely with patients/residents, families, and significant others.
  • Provide education and support to patients/residents, families, and significant others to improve quality of life and promote independence.
  • Perform discharge planning activities, including family/patient education, nursing center documentation, and referrals to community-based agencies and services.
  • Establish and maintain relationships with community-based agencies and services to benefit our patient/resident population.
Requirements
  • Bachelor's degree in social work or a human services field.
  • Any required certifications/licensures as per the State of employment to practice in long-term care.
