As a Social Services Coordinator in a nursing facility, you will play a crucial role in enhancing the lives of patients and their families. Your primary responsibility will be to assess the strengths and needs of individuals, focusing on their social, emotional, and mental health requirements, and to facilitate access to appropriate services.
Objective:
The aim is to ensure that patients achieve the highest possible level of physical, mental, and psychosocial well-being, ultimately enhancing their quality of life.
Key Responsibilities:
- Collaborate with the interdisciplinary team to uphold and advocate for patient rights and overall well-being.
- Educate staff on the significance of social services in patient care.
- Participate in crafting a comprehensive, interdisciplinary care plan tailored to each patient’s social service needs.
- Conduct meetings with patients, families, and significant others to provide support and share information aimed at improving quality of life.
- Engage in discharge planning activities, including family education, documentation, and referrals to suitable agencies and suppliers.
