As a Clinical Team Lead at Northwestern Medicine, you will play a critical role in supporting the Community Support Team in providing high-quality mental health services to clients. This position requires a strong background in assessment, individual, family, and group counseling, as well as excellent leadership and communication skills.

Key Responsibilities
  • Conduct regular quality assurance reviews of CST records with the assistance of a LPHA.
  • Conduct Integrated Assessment and Treatment Plan (IATP) & Level of Care Utilization System (LOCUS) assessments in accordance with Rule 132/140 standards.
  • Complete clinical chart reviews for assigned CST caseloads or client referrals.
  • Manage schedules, appointments, phone calls, paperwork, trainings, meetings, and tracking travel.
  • Document patient medical records in an accurate and timely fashion.
  • Conduct community-based sessions to support clients' progress towards identified treatment goals.
  • Provide crisis intervention and safety planning, including linkage to other teams/facilities as needed.
  • Conduct team meetings, huddles, and supervisions as assigned.
  • Provide linkage and referral to internal and external interventions/resources/services as applicable.
  • Track client data on a monthly basis to maintain program fidelity.
  • Obtain preauthorization/reauthorization from insurance as needed.
  • Maintain Leader on Duty shifts as assigned each month.
Requirements
  • 5 years of experience and training in assessment, individual, family, and group counseling.
  • Master's degree in social work, psychology, counseling, or related field.
  • Master's degree with LSW, LCSW, LPC, or LCPC, or eligible for licensure.
