Position: Hospice Registered Nurse Preceptor/Admissions

Location: Serving Charleston, Berkeley, and Dorchester Counties

As a Hospice Registered Nurse Preceptor/Admissions, you will embody the essence of compassionate care while demonstrating your nursing expertise. Your role is pivotal in delivering outstanding hospice services to patients and their families, ensuring adherence to the highest standards of nursing practice.

Key Responsibilities:

  • Manage a designated case load while providing education and mentorship to hospice clinical staff.
  • Engage in the recruitment and onboarding of new clinical personnel, which includes conducting interviews, developing orientation schedules, and facilitating educational training sessions.
  • Conduct comprehensive assessments of patients and families to identify hospice needs, focusing on both medical history and physical evaluations.
  • Participate in on-call responsibilities as outlined by the office policy.
  • Perform ongoing evaluations of patients' physical and psychosocial conditions.
  • Collaborate with the Interdisciplinary Group (IDG), patients, families, and attending physicians to formulate and implement care plans.
  • Adjust care plans as necessary based on the evolving needs of patients and families.
  • Coordinate care among IDG members and external healthcare providers to ensure comprehensive service delivery.
  • Assess and coordinate additional services required by patients and families in line with the IDG care plan.
  • Supervise the activities of LPNs, Hospice Aides, and Homemakers assigned to patients and families.
  • Facilitate transitions to different levels of care when required.
  • Handle personal health information (PHI) responsibly in accordance with organizational policies.

Qualifications:

  • Graduate from an accredited nursing program.
  • Active RN license in South Carolina, in good standing.
  • Minimum of twelve months of nursing experience within the last five years, or current clinical knowledge as determined by the DHCS.
  • If a recent graduate, must have 1200 verified hours of nursing experience within the last twelve months or completion of a company-approved Preceptor Program.
  • At least one year of experience in hospice care.
  • Preferred one year of experience in a precepting or training role.
  • Current CPR certification.
  • Valid driver's license and auto liability insurance required.
