We are seeking a compassionate and skilled Hospice Registered Nurse to join our team at Interim HealthCare - North Charleston, SC. As a Hospice RN, you will play a vital role in providing high-quality patient care and support to individuals and their families during a challenging time.
Key Responsibilities- Provide home-based care to patients with six months or less to live, focusing on quality of life and comfort.
- Work collaboratively with an interdisciplinary team to develop and implement patient care plans.
- Perform patient assessments, monitor vital signs, and administer prescribed medications and treatments.
- Ensure accessibility of medical equipment, oxygen, medication, and other supplies.
- Educate patients and their families on prognosis, care plan changes, medication, and hospice goals.
- Take a holistic approach to care, engaging patients' minds, bodies, spirits, and families.
- Graduate of an accredited nursing program and active RN license in SC or Compact License.
- Minimum of 5 years of nursing experience, with 2 years of hospice experience.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing specialty, strong communication skills, and compassion.
- Participate in required on-call shifts, including evenings, nights, and weekends as needed.
