As a Hospice Registered Nurse Preceptor/Admissions, you will embody compassion and professionalism while delivering outstanding hospice services to patients and their families. Your role is crucial in maintaining high nursing standards and providing essential education and training to clinical staff.
Key Responsibilities:
- Manage a designated case load while providing education and mentorship to hospice clinical personnel.
- Engage in the recruitment and onboarding process for new clinical staff, which includes conducting interviews, creating orientation schedules, and overseeing training sessions.
- Conduct comprehensive assessments of patients and families to identify hospice care needs, focusing on medical history and physical evaluations.
- Participate in on-call duties as per the established office policies.
- Perform ongoing evaluations of the patient's physical and psychosocial conditions.
- Collaborate with the Interdisciplinary Group (IDG), patients, families, and attending physicians to formulate and adjust care plans.
- Ensure the coordination of care among IDG members and external healthcare providers to meet the evolving needs of patients and families.
- Supervise and direct the services provided by LPNs, Hospice Aides, and Homemakers assigned to patients.
- Facilitate transitions to other levels of care when necessary, based on patient and family needs.
- Maintain confidentiality and handle personal health information (PHI) in accordance with organizational policies.
Qualifications:
- Graduate from an accredited nursing program.
- Active RN license in South Carolina, in good standing.
- At least 12 months of nursing experience within the last five years, or current clinical knowledge if no recent experience.
- If a recent graduate, must have 1200 verified hours of nursing experience or completion of a Preceptor Program.
- Minimum of one year of experience in hospice care.
- Preferred one year of experience in training or precepting.
- Current CPR certification.
- Valid driver's license and auto liability insurance required.
