Role Overview
As we broaden our services in various markets, we seek a Care Transition Navigator who embodies our core values: Trustworthiness, Capability, Compassion, Proactivity, and a Calling to serve patients in their homes. The Care Transition Navigator serves as the representative of VitalCaring when engaging with referral sources, ensuring a seamless transition for patients returning home. Your efforts will position VitalCaring as the preferred partner for compassionate and personalized home health care.
Key Responsibilities
- Build and maintain relationships with referral sources, patients, and their families.
- Work closely with facility discharge planners to facilitate timely and effective discharges.
- Engage directly with patients and families to address all post-transition needs for a safe return home.
- Ensure effective communication among care team members to enhance the transition process.
- Provide exemplary care to patients throughout the care continuum.
- Collaborate with operational leaders to foster a strong team culture, tackle challenges, promote accountability, and drive continuous improvement.
- Utilize market knowledge and available tools to identify growth opportunities.
- Achieve monthly individual performance targets consistently.
