About the Role
- Provide direct nursing care to patients and families in accordance with the scope of the LPN as defined by the North Carolina Board of Nursing.
- Assume responsibilities for patient assessment, development of individualized care plans, and implementation of interventions to achieve optimal patient outcomes.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
- Communicate effectively with patients, families, and healthcare team members to ensure informed decision-making and optimal patient care.
- Participate in ongoing education and professional development to stay current with best practices and evidence-based care.
Requirements
- Graduation from an accredited Practical Nurse program.
- Highly preferred experience in Home Health Setting for 1+ years, or high acuity experience to include wound vac experience.
- Current State of North Carolina LPN licensure is required or Multi-State Compact License (application for permanent NC LPN license required within 90 days of hire).
- Active and current driver's license.
- BLS Certification is required (If expired, this training will be provided during orientation).
