Franciscan Missionaries of Our Lady Health System is a nonprofit organization dedicated to providing high-quality healthcare services to seniors. Our Program of All-Inclusive Care for the Elderly (PACE) is a comprehensive healthcare program designed to assist seniors who want to remain in their communities.

Job Summary

The PACE Center Manager is responsible for the effective operations of the PACE Center, coordinating and supervising all participant services. This role requires strong leadership and communication skills, as well as the ability to work effectively in a team environment.

Responsibilities
  1. Coordination of Participant Care
    1. Coordinates all center activities, including care planning and implementation, preparation for Interdisciplinary Conferences and daily meetings, family conferences, and all related follow-ups.
    2. Coordinates all activities relating to new enrollees.
    3. Communicates and coordinates all participant changes to team members.
    4. Participates in the development of the IDT Plan of Care and is a part of the enrollment processes.
    5. Participates in the coordination of a 24-hour care delivery and manages and participates in the 24/7 On Call Process.
  2. Regulatory Standards
    1. Ensures that established CMS/DHH regulations and standards are met. Understands PACE Regulations and Level 11 Reporting and ensures that PACE staff understands the Regulations.
    2. Responsible for human resource management of all disciplines, including RNs, LPNs, Therapists, Dietary, Pharmacy Staff, Clerical staff, Adult Care Assistants, Recreational Therapy staff, MSW's, and Transportation personnel.
    3. Works closely with the Director in the development and on-going monitoring of contract services, including selection of providers and on-going evaluation of quality of services.
    4. Responsible for the Participant Advisory Committee as required by standards. Ensures quarterly meetings take place and coordinates the selection of committee members and chairpersons.
    5. Responsible for the coordination of grievances according to standards and meeting the time frames required.
  3. Leadership
    1. Serves as a liaison between individual program disciplines, other PACE departments, and community service providers.
    2. Implements effective communication between clinical staff, non-clinical staff, and medical staff to provide the best of care and services to the Participants.
    3. Participates in the Management team of PACE and effectively communicates any changes, new standards, and rationale to the staff.
    4. Participates in the Management of the PACE Budget.
  4. Medical Record
    1. Responsible for knowledge and administration of the electronic medical record and serves as a Super User for the EMR.
