Job Summary

We are seeking a dedicated and compassionate Community Support Specialist to join our team at North Suffolk Community Services Inc. As an Outreach Specialist - Community Support Specialist, you will play a crucial role in the engagement, assessment, and treatment planning process for individuals served.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and their support networks. A strong understanding of mental health principles and practices is essential for this role.

Key Responsibilities:

  • Engage individuals in a person-centered approach to address their needs and goals.
  • Collaborate with a multidisciplinary team to develop and implement individualized treatment plans.
  • Provide side-by-side support and coaching to promote socialization and skill-building.
  • Organize and lead individual and group activities to enhance overall well-being.

Compensation Package: This full-time position offers a competitive salary of $51,000, with opportunities for professional growth and development.

Requirements:

  • High School Diploma or equivalent required; Bachelor's Degree preferred.
  • At least one year of experience working with individuals with severe and persistent mental illness or substance use disorder.
  • MAP and CPR/First Aid certification within five months of hire.
