Position Overview
The Patient Care Coordinator at Marathon Health Centers plays a vital role in assisting patients in achieving their personal health improvement objectives. This position requires the ability to quickly adapt to the patient's culture, environment, and aspirations.

Building relationships based on trust and understanding the patient's health benefits program, as well as the local healthcare community, is essential.

Key Responsibilities
The Patient Care Coordinator will:

  • Engage patients in their health improvement journey and provide care coordination services.
  • Act as a resource and advocate for patient healthcare needs.
  • Manage and develop care coordination services for patients.
  • Coordinate the integration of care management functions with external service organizations and healthcare facilities.

This role involves spending approximately 20% of the time on clinically related tasks and 80% on administrative and patient benefit-related services.

Essential Duties
In collaboration with the clinical team, the Patient Care Coordinator will:

  • Plan pre-visit workflows to ensure care completion prior to patient visits.
  • Coordinate care with hospitals, emergency rooms, consulting physicians, and community resources.
  • Facilitate smooth transitions for patients treated at other facilities.
  • Provide follow-up and coordination with extended care teams.
  • Educate patients about self-management tasks and chronic disease management.

Qualifications
The ideal candidate will possess:

  • An associate degree in healthcare or a related field.
  • A minimum of three years of experience in utilization review, case management, outpatient primary care, or care coordination.
  • Bilingual proficiency in Spanish is required.
  • Experience working with diverse populations, including migrant agricultural workers, is preferred.
