Key Responsibilities:
  • Assist residents in acclimating to the facility's environment
  • Ensure the highest level of psychosocial well-being
  • Facilitate discharge planning as required
Required Qualifications:
  • Bachelor's degree in Psychology, Sociology, Social Work, or LCSW certification
  • Minimum of 2 years of experience in social work within a long-term care setting preferred
Job Requirements:
  • Physically and mentally fit to perform job responsibilities
  • Understanding of the dynamics of the geriatric population
  • Empathy and support for residents
  • Professional and approachable demeanor
  • Compassionate attitude towards the elderly

Main Duties:

  • Orient new residents to the facility

  • Conduct family meetings and foster community relationships
  • Coordinate hospice care when necessary
  • Act as admissions director when required
  • Uphold resident rights and departmental standards
