Amorem is seeking a skilled Clinical Social Worker to join our team as a Hospice Care Specialist. This rewarding role involves providing compassionate support to patients and their families, navigating complex healthcare systems, and promoting holistic well-being.

About the Role

This position requires a deep understanding of hospice care principles, strong communication skills, and the ability to work collaboratively with interdisciplinary teams. As a Hospice Care Specialist, you will be responsible for assessing patient needs, developing personalized care plans, and ensuring seamless transitions between care settings.

Key Responsibilities
  • Evaluate patient needs and develop comprehensive care plans in collaboration with interdisciplinary teams.
  • Assess financial resources and guide patients/families on coverage options for hospice services.
  • Promote patient autonomy and self-advocacy through education and support.
  • Maintain accurate and timely documentation, adhering to regulatory requirements and Amorem's policies.
  • Collaborate with volunteers, chaplains, and other healthcare professionals to ensure holistic care.
Requirements
  • Bachelor's degree in social work or related field; master's degree preferred.
  • At least two years of clinical experience, preferably in hospice care or a related setting.
  • Strong knowledge of hospice philosophy, Medicare/Medicaid regulations, and private insurance coverage.
  • Excellent communication, interpersonal, and problem-solving skills.
