Job Summary:

We are seeking a skilled Social Work Specialist to join our team at Amorem, where you will play a vital role in providing medical social work services to patients and their families. As a key member of our interdisciplinary team, you will be responsible for assessing the physical, financial, psychological, spiritual, and pre-bereavement needs of our patients.

About the Role:

  • You will evaluate the appropriateness of Hospice services for patients and provide education on coverage options, including Hospice Medicare/Medicaid benefit, Medicare, and private insurance.
  • You will develop plans of care with goals defined by patients/families and our interdisciplinary team, providing social work services according to patient plan of care.
  • You will communicate patient changes or family needs to other team members, apprising the primary team of any updates.
  • You will participate in quality improvement programs and adhere to strict confidentiality policies.
  • You will assist in orienting new team members, as requested, and cooperate with government agencies and outside auditors.

Requirements:

  • You must have a Bachelor's degree in social work from a school accredited by the Council of Social Work Education (preferred) or a related field.
  • A minimum of two years of clinical experience is preferred, with one year of social work experience in a healthcare setting being advantageous.
  • You should possess knowledge of and commitment to the hospice philosophy of care, with ability to work effectively on an interdisciplinary team.
  • You will need to complete an initial and ongoing training program, obtain 12 hours of continuing education per year, and attend staff meetings.
