Medical Case Manager

We are seeking a compassionate and skilled Medical Case Manager to join our team at AMOREM. In this role, you will work closely with patients and their families to provide emotional and practical support throughout the hospice journey.

The successful candidate will have a Bachelor's degree in social work and at least two years of clinical experience. They will be responsible for assessing patient needs, developing personalized care plans, and coordinating services with healthcare providers.

• Evaluate patient appropriateness for hospice services and refer to community agencies as needed
• Assess patient financial resources and instruct on coverage for hospice services
• Develop and implement individualized care plans with goals defined by the patient and interdisciplinary team
• Provide ongoing emotional and practical support to patients and their families
• Collaborate with other healthcare professionals to ensure comprehensive care
• Document patient progress and updates in a timely manner

• Bachelor's degree in social work from an accredited program or related field
• At least two years of clinical experience in social work or a related field
• Knowledge of hospice philosophy and policies
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

• A competitive salary range: $65,000 - $80,000 per year, depending on experience
