Job Summary

VCU Health is seeking a highly skilled and compassionate Patient Care Technician to join our Hospital at Home team. As a Patient Care Technician, you will provide direct inpatient-level care to patients in the comfort of their own homes, working closely with our nursing and medical visiting teams to ensure the best possible outcomes.

Key Responsibilities
  • Provide direct care to patients, including vital sign checks, education on in-home technology, and assistance with activities of daily living.
  • Support the virtual command center by screening phone calls, managing medical supplies, and completing outreach to potential remote patient monitoring patients.
  • Work collaboratively with the nursing and medical visiting teams to ensure seamless care coordination and patient satisfaction.
  • Participate in ongoing education and training to stay up-to-date on the latest best practices and technologies in home health care.
Requirements
  • High school degree or equivalent required; Certified Nursing Assistant or equivalent experience preferred.
  • AHA BLS HCP Certification or equivalent required.
  • Ability to work in a fast-paced environment with frequent and intense customer interactions.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
