Job Summary

The Trans Care Navigator provides specialized support to transgender and gender-expansive medical patients of Whitman-Walker Health to promote engagement in healthcare. This role focuses on facilitating referrals that lead to specialty care for gender-related surgeries and other affirming practices.

Key Responsibilities
  1. Engage patients in medical care to meet their goals, address health disparities around HIV, and bridge gaps in access to care.
  2. Liaise between multiple departments within Whitman-Walker and external organizations and health systems to ensure collaborative service provision.
  3. Coordinate activities within the multi-disciplinary Care Team of medical, behavioral health providers, legal services, benefits navigators, and outside specialists to ensure quality service delivery.
  4. Participate in quality improvement (QI) activities to improve health outcomes and data reconciliation.
  5. Develop individualized care plans that are patient-centered and address barriers to care.
  6. Maintain up-to-date and accurate records in the electronic medical record (EMR) system or other data systems.
  7. Follow-up with patients at risk of falling out of care or who have not been recently seen by the practice.
  8. Engage in Quality Initiatives to improve documentation and care for transgender and gender-expansive clients.
  9. Maintain a collaborative list of resources and gender-affirming specialty medical providers.
  10. Support the development of educational materials, including patient-facing handouts and social media campaigns.
Requirements
  • Understanding of and sensitivity to unique health and wellness needs of transgender and gender-expansive communities and those with HIV-related concerns.
  • Knowledge of health disparities within transgender communities and reasons for disengagement from health systems.
  • Knowledge of best practices engaging groups that have experienced harm and marginalization within health systems.
  • Knowledge of techniques for assessing patients' needs, developing risk-reduction and referral plans, identifying resources, making referrals, and following up to ensure completed referrals.
  • Fluency in medical terminology and electronic medical records is strongly preferred.
  • Ability to travel with patients in and around assigned geographic areas using public transportation.
Preferred Qualifications
  • Minimum two or more years of paid work experience providing direct assistance to transgender and gender-expansive communities in public health, healthcare, human services, or other community settings.
  • Experience with providing HIV prevention, linkage, or retention services preferred.
  • Written and oral fluency in Spanish or Amharic required.
  • Two years of experience working with a broadly diverse population and the ability to work harmoniously with diverse groups of individuals required.
