We are seeking a highly skilled and compassionate Nurse Navigator to join our team at Community of Hope. As a Nurse Navigator, you will play a critical role in ensuring that patients receive comprehensive coordination of care, including transition of care planning, patient education, and social service referrals and linkages, in a timely and culturally appropriate manner.
Key Responsibilities:- Develop and implement individualized care plans that address the unique needs of each patient.
- Coordinate with healthcare providers, social services, and community resources to ensure seamless transitions of care.
- Provide patient education and support to empower patients to take an active role in their healthcare.
- Collaborate with the care team to ensure that patients receive high-quality, patient-centered care.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Bachelor of Science in Nursing degree required.
- Current, unencumbered DC Registered Nurse license with current CPR certification required.
- Knowledge of primary care and health maintenance required.
- Experience with educating patients with chronic health conditions preferred.
- Ability to work with computers and electronic health records required.
- Strong verbal and written communication skills required.
- Strong organizational skills with an ability to multitask required.
- Competitive salary range of $95,000 to $105,000 annually.
