Job Summary

We are seeking a highly skilled and compassionate Case Manager/Resource Coordinator to join our team at Wedgwood Christian Services. As a key member of our community outreach team, you will be responsible for providing intensive case management services to assigned clients, coordinating resources between multiple agencies, and promoting our program to generate new referrals and awareness.

Key Responsibilities
  • Case Management: Develop and implement case management plans for each client, working collaboratively with the Clinical Supervisor and other team members as needed. Prioritize client needs by seeking feedback from all involved parties.
  • Community Outreach: Facilitate multiple case aspects to meet the health and human service needs of the client. Develop and maintain a resource listing for the applicable service area.
  • Program Promotion: Make routine visits to referral sources to generate referrals and promote our program. Serve as the lead community expert for promoting and marketing our services in assigned geographical areas.
  • Quality Assurance: Track information, beginning at intake and admission, through treatment and into aftercare, which becomes part of our statistical data bank. Ensure that all authorizations/reauthorizations or documentation/paperwork requirements are clinically appropriate and timely for all county, State of Michigan, Court, and Managed Care Agencies.
  • Client Discharge: Coordinate aftercare plans to ensure treatment continuity for the clients' psychiatric, medical, and therapeutic care.
  • Professional Growth: Participate in educational opportunities and trainings provided by the agency and through CMH that meet program requirements for continued development of skills and clinical expertise.
Requirements
  • Bachelor's degree in Social Work, Psychology, or another associated human service profession required.
  • Experience working with adolescents, women, and/or families preferred.
  • Subscription to the Statement of Faith of Wedgwood Christian Services and agreement with the goal, purpose, and objectives of the agency.
